AI-driven Patient Journey Analysis for Enhanced Care Coordination and Continuity
Abstract
This paper introduces an AI-driven approach to analyze patient journeys across healthcare settings to improve care coordination and continuity. By leveraging machine learning algorithms, our system integrates data from electronic health records, claims databases, and care management platforms to identify gaps in care, predict care transitions, and optimize care pathways. Implementation of this approach facilitates seamless transitions between healthcare providers, reduces fragmentation of care, and enhances the overall patient experience.
