Abstract

Chronic diseases pose significant challenges to healthcare systems globally. This paper investigates how artificial intelligence (AI) can transform chronic disease management through predictive analytics, remote monitoring, and personalized treatment plans. By analyzing patient data in real-time, AI can identify early warning signs and recommend interventions to prevent disease progression. We review successful implementations in diabetes, heart disease, and respiratory conditions, highlighting improvements in patient adherence and outcomes. The paper also discusses barriers to adoption, including data integration and patient privacy concerns, proposing solutions to enhance AI's role in proactive healthcare.
